About Your Rights
You should consider contact with us as an attempt to collect a debt as we sometimes act as a debt collector. Any information provided by you will be used to collect this debt. However, if you are in bankruptcy or received a bankruptcy discharge of this debt, any information obtained from you will be maintained for informational purposes only.

If you notify us that you do not want to be contacted by telephone at your place of employment, we will not contact you there.

If you notify us that you refuse to pay the debt or that you wish us to cease further communication with you, we will not communicate with you except to advise you that our efforts are being terminated, that we may invoke specific remedies ordinarily invoked by collection agencies, and, if applicable, that we intend to take a specific remedy permitted by law.

We may report information about your account to credit bureaus. Late payments, missed payments, or other defaults on your account may be reflected in your credit report.

A WRITTEN APPLICATION FOR REIMBURSEMENT FROM THE RECOVERY FUND MUST BE FILED WITH AND INVESTIGATED BY THE DEPARTMENT PRIOR TO THE PAYMENT OF A CLAIM.
